Transitioning to a new environment can be stressful. At Peak Resources, we want the process of entering one of our resident communities to feel like you are entering a caring and compassionate home. In an effort to keep our resident’s and their families focused on the road to recovery, we’ve put resources in place to meet expectations and individual healthcare goals. Our skilled 24-hour nursing staff is available to provide that individual care you or your loved one needs.
During Your Stay
Each of our facilities has a dedicated Admissions team consisting of an Administrator, Admissions Director, Director of Nursing, Social Worker and other designated clinical staff who will transition you or your loved into our care. There will be routine coordinated care plans to access the individual needs of each of our residents.
We encourage communication during and after your stay. Please share any comments or concerns you have about your clinical needs. Each member of our team is available to answer your questions. Thank you for the confidence you have placed in Peak Resources Inc. to manage your medical care.
Our Admissions Team will meet with you and/or your family prior to or during the admissions process to gather important information. In order for us to develop your clinical plan and ensure proper billing, it is very important that we obtain several key documents from you.
Please be prepared to provide us with the following items prior to or during the admissions process:
- Medicare card
- Medicaid card
- Managed care or insurance card
- Social Security card
- Medicare Part D (drug benefit) card
- Advance directives, living will, etc. (if applicable)
- Clinical and/or financial Power of Attorney documents
- Long-term care/supplemental insurance policies
- Name and contact information of the person to be contacted with change in condition/financial status
Our admissions personnel review all of the required documents, ensure that you have proper copies of all paperwork, also explain programs and services provided in our nursing and rehabilitation facility, and answer any questions that you may still have. You may also meet with a member of our business office to review any finance-related matters and/or questions.
As a skilled nursing and rehabilitation company, Peak Resources offers several payment options. Payment options include: Medicaid, Medicare, Private-Pay, Private/Supplemental Insurance.
Our business office and admissions department can provide detailed assistance in determining the most appropriate form of payment. We are committed to providing the optimum level of patient care at the most reasonable cost to the resident. We believe this can best be attained within the framework of sound fiscal management. In order to create and uphold an atmosphere of mutual understanding with the communities we serve, we work closely with residents and families to ensure that financial responsibilities have been taken care of prior to admission.
